FROM THE SCHOOL NURSE- A REMINDER TO 4TH GRADE PARENTS

Massachusetts State law requires that all children in the fourth, seventh and tenth grade have a physical examination. Your child’s primary care physician should perform this physical and you should forward written documentation of an exam done after June 2010 to your child’s school.

If you already have the required paperwork at home please send it in to your school as soon as possible.

Please note – If you have difficulty scheduling an appointment with your child’s physician within the allotted time, please contact me at the Woodville School and I’ll make arrangements for our school physician, Dr. Kidd, to perform the physical exam when he visits our school in May (exact date not yet set).

PLEASE REMEMBER - A copy of a physical must be on file for your child to enter grade 5 at the Galvin Middle School in September. If the physical is to be done over this coming summer, please forward it to the Health Office at the Galvin before the first day of school. Thank-you!
